Clearance to Fly (Fit-to-fly Letter)
Hope Air requires a letter from the hospital confirming that a patient is fit to fly home if the following circumstances apply:
- When a patient was airlifted to the hospital and will be discharged to return home.
- When Hope Air has flown a patient one-way to their medical appointment without knowing their return date.
When a discharge date has been confirmed, please submit a travel request on our website and email the letter (written on letterhead) to info@hopeair.ca. The letter can be provided by a nurse or social worker.
Please include the following information:
- Patient’s name
- Date of admission
- Reason for admission (i.e. surgery, procedure)
- Confirmed discharge date
- In addition to confirming clearance to fly, please also confirm if an escort is medically required
NOTE: We appreciate as much notice as possible to arrange travel. If you have not flown with Hope Air on an inbound flight, 2 business days are required to process applications.
